Our holistic team provides support in three distinct areas.


Our Medical Directors, Nurses, and Certified Home Health Aides work together to create and implement a customized care plan for the patient. They coordinate regularly with the attending physician and primary caregiver to reassess the patient’s changing needs regularly.

  • 24-hour on-call nursing services

  • Medications related to hospice diagnosis, pain
    management, and symptom control

  • Medical equipment and care supplies related to 
    hospice diagnosis

  • PT/OT in home

  • Dietary counseling

  • Care plan coaching for the patient and family

  • Personal care by certified home health aides

  • Wound care and wound care supplies

  • High flow oxygen (up to 10L, per 
    compliance regulations)

  • Visit from Registered Nurse twice weekly

  • Visit from Home Health Aide three times weekly

  • Increased visit frequency as needed

Emotional & Spiritual

Our Chaplain and Volunteers provide support, counseling, and companionship to our patients and their families.

  • Respite support

  • Spiritual counseling 

  • End-of-life companionship

  • Bereavement support


Our Social Worker assesses patient and family needs and develops a plan to meet those needs, including utilizing any and all available resources.

  • Direct counseling

  • Referrals to appropriate resources 
    and community agencies
